In the past 5 years, have you been diagnosed with or treated for cancer, heart attack, or stroke?
Do you use a wheelchair or motorized mobility device?
Do you have COPD, or do you use a nebulizer or oxygen?
Do you have any pending tests, treatments, therapies, surgeries not yet performed, or are you currently undergoing physical therapy?
Do you have diabetes along with complications such as neuropathy?
